Membership Eligibility:
Anyone who lives or works in any of the following Florida counties: Leon, Baker, Broward, Clay, Duval, Flagler, Orange, Hillsborough, Miami-Dade, Seminole, Nassau, and St. Johns is eligible to open an account. Additionally, employees of CSX Transportation and other specified companies are also eligible to join.

Before you begin, below are a few helpful tips for filling out this form:

The approximate time needed to complete the application is up to 20 minutes.

• To move quickly through the process, please have the following important information ready for all owners and beneficiaries on the account:

• The membership application cannot be “saved”, and started over. If you “quit” the application or the “session expires” (after 15 minutes of inactivity) – you must start a new application again - from the beginning.

• You’ll be required to print a “Signature Card” at the end of completing the application. If you cannot print the signature card, then download it and print it at a later time.
